Job summary

BRC is seeking a dedicated case manager to deliver comprehensive services to chronically homeless individuals in New York. You will manage caseloads, conduct assessments, help overcome housing barriers, and coordinate entitlements within a busy program setting.

The role requires strong communication, writing, and computer skills, with experience in residential treatment and homeless populations. CPR training or willingness to obtain training is required.

Qualifications

• High School Diploma or GED required; bachelor's degree preferred and master's ideal.
• Bilingual Spanish strongly preferred, but not required.
• Excellent communication and writing skills and computer proficiency required.
• Experience with residential treatment and with homeless populations.
• CPR training certification or willingness to take CPR training.

Responsibilities

• Provide comprehensive case management to identified chronic homeless individuals.
• Manage all aspects of assigned caseload with regular client meetings in field and office.
• Conduct comprehensive assessments to identify housing barriers and assist with entitlements.
• Develop and progress individualized housing plans and coordinate support services.
• Prepare clients for housing interviews and arrange housing appointments.
• Liaise with clinical staff in BRC or external programs regarding common clients.
• Provide emergency first aid/CPR or overdose prevention assistance when needed.
• Related duties as assigned.
