Job summary

BRC is seeking a dedicated Case Manager in Manhattan, NY, to provide comprehensive services for chronically homeless individuals. You will manage a caseload, conduct assessments, and help clients overcome housing barriers while developing individual housing plans and coordinating with clinical staff.

The role requires strong communication, writing, and computer skills; a High School Diploma or GED is required with a Bachelor's or Master’s degree preferred. CPR training is a plus.

Qualifications

• High School Diploma or GED required; Bachelor's degree preferred and Master's ideal.
• Bilingual Spanish strongly preferred but not required.
• Excellent communication and writing skills and computer proficiency required.
• Experience in residential treatment and with homeless populations.

Responsibilities

• Provide comprehensive case management to identified chronic homeless individuals.
• Meet clients regularly in the field and office; maintain up-to-date clinical files and reports.
• Assess barriers to housing; assist in securing entitlements and progressing housing plans.
• Liaise with clinical staff and provide CPR/first aid as needed.
