Job summary

BRC is seeking a dedicated Case Manager for the Transit Homeless Outreach program in New York City. The role focuses on delivering comprehensive case management to chronically homeless individuals, coordinating housing-related services and entitlements, and working across on-site and field settings to support clients toward stable housing.

You will conduct assessments, develop housing plans, and collaborate with clinical teams to connect clients to mental health and substance abuse resources,

Qualifications

• BA in related field preferred.
• Significant outreach or case management experience strongly preferred.
• Experience referring clients to entitlement programs, mental health programs, substance abuse programs, and work readiness programs.
• Ability to assess signs and symptoms of mental illness and substance abuse; good writing and oral communication skills.
• Computer literacy required; bilingual preferred including Spanish, Polish, Russian, Chinese, Korean or other language.
• Must have valid state driver's license; CPR training certification or willingness to take training.

Responsibilities

• Provide comprehensive case management services to identified chronic homeless individuals.
• Meet with clients regularly in the field and office; ensure clinical files and reports are complete and up to date.
• Conduct comprehensive assessments to identify barriers to housing and assist clients in securing entitlements.
• Develop and progress individualized housing plans; build a network of support services.
• Prepare clients for housing interviews and arrange housing appointments; participate in case conferences as needed.
• Liaise with clinical staff in BRC or partner programs regarding shared clients; ensure coordination.
