Job summary

BRC in New York City seeks a dedicated case manager to provide comprehensive services to chronically homeless individuals. You will manage a caseload, meet clients in the field and office, complete clinical files, and coordinate housing plans while addressing barriers to housing.

Qualifications

• High School Diploma/GED required; Bachelor's degree preferred; Master's ideal.
• Bilingual Spanish strongly preferred but not required.
• Excellent communication and writing skills and computer proficiency required.
• Experience with residential treatment and with homeless populations.

Responsibilities

• Provide comprehensive case management to identified chronic homeless individuals.
• Meet with clients regularly in field and office; maintain complete and up-to-date clinical files and reports.
• Conduct comprehensive assessments to identify housing barriers and assist clients to overcome them.
• Develop individualized housing plans and coordinate with support services to progress housing readiness.
• Prepare clients for housing interviews and arrange housing appointments; participate in case conferences as needed.
• Liaise with clinical staff in BRC or external programs regarding common clients.
• Provide CPR/first aid guidance or overdose prevention as needed.
