Catholic Charities Comm is seeking a dedicated Housing Case Manager to support clients experiencing homelessness by linking them to safe, stable housing and essential resources. You will build relationships, navigate housing applications, and provide ongoing support to promote self-sufficiency.

Join a collaborative Housing team focused on coordinated entry, documentation, and outreach with community partners to achieve client goals and outcomes.

Qualifications

• Minimum of a High School Diploma or Equivalent with 2 years of relevant work experience.
• Bachelor’s degree in Social Work or related field preferred; bilingual Spanish preferred.

Responsibilities

• Provide case management and client advocacy for Housing Programs.
• Engage clients within 48 hours via home visits or calls; document all contacts.
• Assist with coordination entry, housing access points, and plan development.
• Conduct intake assessments for shelter and housing programs; assess eligibility.
• Maintain records, reports, and monthly progress notes; assist with billing.
• Collaborate with community organizations and government agencies; provide resources.
