Telecare Corporation in Santa Ana, CA seeks a compassionate Case Manager/Housing Specialist to support clients with mental illness and co-occurring disorders, helping them live independently in their community. The role requires driving clients to appointments and coordinating housing-related services.

We offer full-time daytime shifts (Tue-Sat) with wage range $23.00–$26.25 per hour, and geographic differentials based on location. Benefits include PTO, holidays, 401(k), and more.

Qualifications

• High School Graduation or GED with 3 years direct services in mental health or community services; OR Associate's degree with 2 years; OR Bachelor's degree in Social Services.
• At least 1 year of direct service experience as Housing Specialist.
• Valid and current driver's license, and personal vehicle insurance in your name.
• Willingness to use personal vehicle to drive clients to appointments and groups, etc.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.

Responsibilities

• Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
