Job summary

Telecare is seeking a Specialty Case Manager - Mental Health in Santa Ana, CA to provide primary case management for adults with serious mental illness. This full-time, on-site role supports independence in the community and collaborates with the treatment team.

Under supervision, you’ll use your specialty knowledge to assist clients and ensure effective service delivery. The wage range starts at $23.00–$26.25 per hour with geographic differentials and competitive benefits.

Qualifications

• High School Graduation or GED with direct services experience.
• Associate's degree with two years of experience or Bachelor's in Social Services.
• At least 1 year of experience in the specialty area (e.g., benefits, education, employment).
• Valid driver's license and insurance; willingness to drive client appointments.

Responsibilities

• Serve as primary case manager for individuals with mental illness.
• Assist clients to live as independently as possible in their community.
• Provide specialty knowledge to the treatment team and program.
• Coordinate transportation for clients when needed.
