Job summary

City Rescue Mission in Oklahoma City, OK is seeking an Experienced Housing Case Manager for the Key to Home Encampment Rehousing Initiative. You will provide comprehensive case management to individuals experiencing chronic homelessness, coordinating with Street Outreach, Landlord Engagement, and Closure Maintenance teams to move clients from encampment to stable housing.

Responsibilities include enrolling clients, assisting with housing applications, delivering weekly in-person case management

Qualifications

• Experience working with chronically homeless individuals.
• Ability to collaborate with outreach, landlords, and housing teams.
• Strong organizational skills and attention to detail.
• Ability to maintain confidentiality and ethical standards.

Responsibilities

• Manage a caseload of up to 20 households transitioning to housing.
• Collaborate with Outreach, Landlord Engagement, and Closure Maintenance teams.
• Assist clients with housing applications, vouchers, and move-ins.
• Provide in-home weekly case management for one year.
• Monitor progress and adapt service plans to housing stability.
