City Rescue Mission seeks a Part-Time Case Manager to join the One Door OKC project on the weekends. You will perform crisis response, family assessments, and housing problem-solving, coordinating with CRM staff and community partners to connect families with safe housing and essential resources.

The role requires compassionate engagement, strong documentation in HMIS, and adherence to agency procedures while maintaining confidentiality and client-centered care.

Qualifikationen

• Experience with families experiencing homelessness, including emergency services, crisis response, assessment, diversion, and case management.
• Familiarity with housing resources, mental health, benefits, transportation, identification, and healthcare services.
• Strong interpersonal communication skills to establish rapport and advocate for clients.
• Ability to learn standardized triage, screening, intake, and safety procedures.
• Ability to work independently, manage multiple priorities, and meet deadlines.
• Maintain confidentiality and adhere to professional ethics and agency policies.
• HMIS experience preferred.
• A valid Oklahoma driver’s license and transportation are required.

• Provide weekend emergency coverage and crisis stabilization within established protocols.
• Conduct weekend triage and family assessments using CRM tools and procedures.
• Coordinate with CRM staff and community resources to access housing and support services.
• Document all interactions and referrals in HMIS and maintain accurate records.
• Arrange transportation and initiate referrals to housing, benefits, and healthcare services.
• Ensure clear handoffs to weekday staff with actions taken and next steps.
• Follow safety, escalation, and incident-reporting procedures; maintain professional boundaries.
