Job summary

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Counseling or related field.

Responsibilities

• Manage ongoing case plans and update as clients meet goals or encounter new barriers.
• Facilitate referrals to medical, mental health, substance use, legal, and employment services.
• Provide direct support in navigating housing placements, lease agreements, and move-in coordination.
• Assist with housing readiness and unit matching using PADMISSION.
• Offer personalized coaching on budgeting, independent living skills, and conflict resolution.
• Prepare correspondence and documentation on behalf of clients as needed.
• Act as an advocate at client appointments, hearings, and program meetings.

Education
