CENTRAL ARIZONA SHELTER SERVICES INC is seeking a Case Manager who will work directly with individuals experiencing homelessness. This role uses a trauma-informed, client-centered approach to assess needs, connect clients with services, and support them toward self-sufficiency and housing stability.

Responsibilities include conducting thorough assessments, developing individualized plans, coordinating referrals, and advocating for clients' access to housing, benefits, and employment resources.

Qualifications

• Associate’s degree in social or behavioral science or related field.
• 1 year of social services experience or equivalent.
• Ability to obtain AZ Level One Fingerprint Clearance Card.
• Proficient in Word, Excel and Outlook; accurate data entry.

Responsabilités

• Conduct in-depth client interviews and orient individuals to the program.
• Evaluate client strengths, needs, barriers and goals; create individualized case plans.
• Provide referrals and guidance to support progress toward goals; update case plans regularly.
• Conduct regular follow-up meetings to support progress and adherence to plans.
• Coordinate with housing providers to expedite placements and ensure housing stability.
• Assist clients with job readiness, including reviewing listings and resumes.
• Enter client information into HMIS promptly after intake; maintain accurate notes.
• Maintain HMIS files and ensure continuity of services through timely documentation.
• Work with team members to ensure coordinated, holistic client support.
• Escalate urgent issues to leadership and participate in meetings and trainings.
