Job summary

CrescentCare in New Orleans seeks a licensed social worker to provide housing case management for disabled individuals in supported housing, aiming to increase housing stability through coordinated services.

You will work with an interdisciplinary team, engage clients several hours daily, document thoroughly, and coordinate with providers and funders to connect resources and crisis planning.

Qualifications

• Licensed social worker (RSW/CSW/LMSW/LCSW) from a Council on Social Work Education-accredited program.
• Bachelor’s degree (BA/BS) required.
• Minimum three years of experience providing social services.
• Reliable transportation, valid driver’s license, with clean record in past three years.

Responsibilities

• Provide housing case management to support housing stability.
• Engage clients approximately 5 hours daily.
• Submit timely, well-written notes and required paperwork.
• Assess client needs and coordinate services with care teams/community partners.
• Develop crisis plans with clients.
