Qualifications

• Bachelor’s degree in a health care field with two years of case management experience in behavioral health.
• Master’s degree in behavioral health may substitute for one year of general experience.
• Connecticut RN license and two years of psychiatric setting experience may substitute for the degree.

Responsibilities

• Implement programs and monitor clients in a supported living program.
• Work as part of an interdisciplinary team on service plans for clients.
• Guide clients to develop independent living skills and support activities of daily living.
• Assist clients with accessing goods, services and supports for independent living.
• Maintain client health and safety and document reports accurately.
• May administer medications with proper certification.
