Qualifications
- Bachelor’s degree in a health care field with two years of case management experience in behavioral health.
- Master’s degree in behavioral health may substitute for one year of general experience.
- Connecticut RN license and two years of psychiatric setting experience may substitute for the degree.
Responsibilities
- Implement programs and monitor clients in a supported living program.
- Work as part of an interdisciplinary team on service plans for clients.
- Guide clients to develop independent living skills and support activities of daily living.
- Assist clients with accessing goods, services and supports for independent living.
- Maintain client health and safety and document reports accurately.
- May administer medications with proper certification.

- 40 Coventry St., Hartford
- 828 Sullivan Ave., South Windsor
Our Addiction Services Division provides a full continuum of care, including:
- Primary care integration
- Residential detox and treatment
- Outpatient mental health and substance use services for adults and children
- Intensive outpatient programs

Accountable for acting as part of a team in the service delivery plan for persons with mental health and or addiction disorders and the care, guidance, supervision and training of clients in a supported or supervised housing program.
- Implements programs and monitors clients within a supported/supervised living program
- Functions as a member of an interdisciplinary team and participates in service delivery plan process for clients
- Provides guidance and support for clients to develop independent living skills
- Participates in and assists clients with activities of daily living
- Assists clients in accessing goods, services and supports required for independent living; reinforces appropriate client behavior through modification techniques
- Responsible for clients health and safety
- Observes and reports client behavior
- Serves as advocate for government subsidies and re-determination hearings
- Transports clients
- Acts as liaison to clients and their families or various community agencies or organizations
- Provides concise and accurate reports and documentation
- May make recommendations on policies or standards
- May administer and track medications with appropriate certification
- May administer first aid in emergency situations
- May schedule and attend appointments with clients
- Performs related duties as required
- All agency staff are required to attend all mandatory department/agency meetings and trainings
Sunday - Thursday, 8:00 AM - 4:30 PM
A Bachelor’s degree in a health care field and two (2) years of experience using case management techniques in a Behavioral Health setting. A Master’s degree in a behavioral health discipline may be substituted for one (1) year of the General Experience. A current Connecticut license as a registered professional nurse and two (2) years of experience working in a psychiatric setting may be substituted for the Bachelor’s degree. On occasion, extensive community mental health experience with seriously and persistently mentally ill persons may be substituted for the degree requirements noted above.
