Job summary

Care New England Health System and its affiliates seek a Residential Case Manager to deliver client-centered, recovery-oriented case management in a residential setting. You will work with a multidisciplinary team to assess needs in housing, education, employment, healthcare, and finances, and to develop individualized care plans with clients and families.

Responsibilities include coordinating treatment plans, crisis intervention when needed, medication support, transportation, and connecting

Qualifications

High School diploma or GED required.
1–3 years in a behavioral health setting; residential experience preferred.
Valid driver’s license, vehicle registration, current inspection and auto insurance.
Ability to travel within the community using reliable transportation.
Bilingual ability preferred.
Proficiency with documentation/EMR systems and accurate record-keeping.

Responsibilities

Coordinate and deliver person-centered, recovery-oriented case management with a multidisciplinary team.
Align case management with clients’ mental health treatment plans.
Conduct comprehensive assessments across housing, education, employment, healthcare, and finances.
Develop, present, and implement individualized care plans with clients and families.
Provide support counseling for mental health, substance use, and medical issues.
