North Central Mental Health Services, Inc. is seeking a Behavioral Health Case Manager to join our Franklin County team. The role focuses on advocacy, coordination, and empowerment within a community-based care model.

You will develop care plans, connect clients to essential services, and monitor progress while ensuring compliance and compassionate service delivery in a busy, supportive environment.

Qualifications

• Bachelor’s Degree Required (Social Work, Psychology, Behavioral Health, or related field preferred).
• Valid Ohio Driver’s License & reliable transportation.
• Completion of 90-Day Paid Training Program.
• Successful Background Check & Pre-Employment Drug Screening.

Responsibilities

• Care Plan Development: Work with clients, families, and healthcare professionals to create personalized treatment plans.
• Service Coordination: Connect clients to therapy, housing, employment support, and recovery programs.
• Advocacy: Champion clients' rights within healthcare and social service systems.
• Ongoing Monitoring: Track progress, ensure treatment adherence, and adjust plans as needed.
• Crisis Intervention: Provide immediate assistance to ensure safety and wellbeing.
• Documentation & Compliance: Maintain accurate EMR records and meet accreditation standards.
• Empowerment & Cultural Sensitivity: Deliver respectful services recognizing diverse backgrounds.
