Job summary

Community Reach Center seeks a dedicated Case Manager to join its ACT Team, delivering integrated behavioral health services and psychosocial rehabilitation support in the community.

You will work closely with the care team to link consumers with resources, provide brief interventions, and track outcomes in the EHR, with opportunities for professional growth and cross-licensure sponsorship as noted in the role description.

Qualifications

• Requires bachelor’s degree in psychology, social work, or directly related field.
• 6 months of experience working with adults who have mental illness preferred but not required.
• Communication, organization, time management and clinical skills.
• Ability to work independently in a community-based setting.
• Valid Colorado Driver’s license with a clean driving record.

Responsibilities

• Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and families as appropriate to the position.
• Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider, supports and family members as needed.
• Provide brief interventions using evidence-based techniques such as Motivational Interviewing or other treatments to support engagement in services, housing, medical appointments and healthy living activities.
• Track patient follow-up and clinical outcomes using EHR; document encounters and re-engage patients as needed.
• Attend collaborative treatment team meetings and reviews of consumers.
