Job summary

BRC is seeking a dedicated case manager to handle a caseload, meeting clients regularly and ensuring all clinical files and reports are current. You will conduct off-site visits, drop off reminders, and support clients through housing applications and housing interviews.

The role emphasizes assisting clients with entitlements, building independent living skills, and liaising with multidisciplinary teams. CPR and computer literacy are required, with a BA preferred and HS diploma acceptable.

Qualifications

• Strong writing and oral communication skills.
• Ability to work in a high stress/volume environment while coordinating with a treatment team.
• Proficiency with basic computer applications and documentation.

Responsibilities

• Meet with clients regularly and ensure clinical files and reports are complete and up to date.
• Make weekly visits to client rooms off site and conduct room inspections.
• Assist clients with entitlements, housing applications, and housing interviews.
