UnitedHealth Group’s Optum Home & Community Care seeks a HouseCalls Nurse Practitioner to perform in-home health assessments for Medicare Advantage and other plans. This field-based, non-prescribing role focuses on preventive care, member education, and closing care gaps with primary care providers.

Responsibilities include comprehensive home visits, medication reconciliation, vitals and screenings, and identifying social determinants of health with referrals as needed.

Formación

• ANCC or AANP certification in Family, Adult, Geriatric or Adult-Gerontology Primary Care.
• Active, unrestricted state licensure or ability to obtain by start date.
• Active, unrestricted driver's license and reliable transportation for home visits.
• Ability to navigate a variety of home environments during assessments.
• Meet state requirements for independent practice approvals where required.

Responsabilidades

• Conduct comprehensive in-home health assessments.
• Review medical history and reconcile medications.
• Complete physical exams, vital signs, screenings and point-of-care testing.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close gaps in care.
• Recognize urgent or emergent situations and respond appropriately.
• Educate members on health conditions, medications and treatment compliance.
• Identify social determinants of health and provide referrals.
