Job summary

UnitedHealth Group's Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve health outcomes.

The position offers a $10,000 sign-on bonus for external candidates, flexible scheduling (8am-8pm, 7 days), and no on-call, weekends, or holidays.

Qualifications

• Active ANCC or AANP certification required.
• Current, unrestricted state license to practice.
• Experience performing in-home health assessments.
• Ability to travel to patient homes for visits.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Identify urgent or emergent situations and intervene appropriately.
• Educate members on disease processes, medications and adherence.
• Address social determinants of health and provide referrals.
