Job summary

Advocate Health in Charlotte, NC, is seeking a part-time Social Worker to join Care Management. The role is 24 hours per week, 7a–7p, with a pay range of $30.70–$46.05 per hour. Responsibilities include psychosocial assessments, discharge planning, and coordinating with hospital staff and social services.

Required is a Master of Social Work; an LCSW is preferred. Hospital experience and community resource knowledge are advantageous. Total Rewards benefits and retirement plans are available.

Qualifications

• Completes psychosocial assessments for post-hospital care.
• Assists patients in coping with stress related to hospitalization.
• Liaises with hospital staff and Department of Social Services.

Responsibilities

• Completes psychosocial assessments for the patient's post hospital care for designated patients and their support system.
• Assists patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
• Works with the Clinical Care Management team to assess and evaluate the patient for the appropriate level of care or environment setting to meet care needs across the continuum.
