Job summary

Methodist Le Bonheur Healthcare is seeking a Social Worker I to coordinate patient-centered care and facilitate transitions across the acute care episode. The role emphasizes psychosocial assessment, discharge planning, resource coordination, and patient/family support without providing psychotherapy or clinical interventions.

You will collaborate with the interdisciplinary team, assist with Medicaid and other benefits applications, and ensure compliant documentation in the EMR.

Qualifications

• Master's Degree in Social Work is required.
• Licensed Master Social Worker in Tennessee or eligible to obtain licensure within 12 months.
• Experience in acute care, healthcare, social services, or community resource coordination preferred.

Responsibilities

• Discharge Planning & Care Coordination with the care team.
• Provide education on resources, benefits, and community programs; support patient/family.
• Identify financial, housing, transportation, and social support needs; connect to resources.
• Participate in daily rounds, huddles, and care coordination meetings.
• Complete timely documentation in the electronic medical record and ensure compliance.
