Regional One Health is seeking a Medical Social Worker (LMSW) to provide psychosocial assessments, counseling, and discharge planning for patients and families in collaboration with the medical team. The role emphasizes documentation, crisis intervention, and coordination with community resources to support complex patient needs.

The position requires a Master’s in Social Work and LMSW licensure in Tennessee, with preferred experience in hospital social work and patient advocacy to promote

Competenze

• Master's Degree in Social Work from an accredited college or university (Required)
• LMSW - Licensed Medical Social Worker - State Licensure in the State of Tennessee Upon Hire (Required)
• Minimum 1 year social work experience (Preferred)

Mansioni

• Perform psychosocial assessments and provide counseling to patients and families.
• Collect information from patient, family, and communities to support assessments.
• Coordinate with health care team to plan discharge and determine care needs.
• Document assessments and plans within 24 business hours of referral.
• Provide crisis intervention and refer to appropriate services as needed.
• Communicate discharge plans and coordinate multi-disciplinary team conferences.
