Job summary

Regional One Health in Memphis, TN is seeking a Medical Social Worker (LMSW) to provide psychosocial assessment, counseling, and discharge planning in coordination with the medical team. The role supports patients and families, ensuring timely documentation and referrals to community resources.

The ideal candidate holds an LMSW license, a Master’s in Social Work, and at least 1 year of social work experience.

Qualifications

• Master's Degree in Social Work from an accredited college or university Required
• LMSW - Licensed Medical Social Worker - State Licensure in the State of Tennessee Upon Hire Required
• Minimum 1 year experience of social work experience Preferred

Responsibilities

• Assess patients and families and implement social work services.
• Determine priorities for social work intervention with emphasis on discharge planning and patient self-determination.
• Coordinate with care teams and community services to initiate discharge planning.
• Provide crisis intervention and supportive counseling to patients and families.
• Document assessments and recommendations within 24 business hours of referral.
