Competenze

• Master's Degree in Social Work from an accredited college or university.
• LMSW - Licensed Medical Social Worker - State Licensure in Tennessee upon hire.
• Minimum 1 year of social work experience preferred.
• Experience with adolescent, adult and/or geriatric patients as assignment dictates.

Mansioni

• Perform psychosocial assessments and provide counseling for patients and families.
• Coordinate discharge planning with multi-disciplinary teams.
• Document assessments and recommendations within 24 business hours of referral.
• Identify high-risk patients and initiate appropriate referrals and services.
• Communicate with community resources to arrange services like DME, home care, and therapy.
• Maintain patient confidentiality per HIPAA and hospital policies.
• Participate in quality improvement and staff training activities.
