The role emphasizes psychosocial assessments, care coordination, referrals to community resources, and collaboration with the Clinical Care Management team. Master of Social Work is required, with LCSW preferred; at least 1 year hospital or health-system experience; benefits eligible; 40 hours per week.

Qualifications

• Master of Social Work required; LCSW preferred.
• Applicable state Certification preferred.
• Adherence to NASW Code of Ethics.
• At least 1 year of hospital or health-related experience preferred.
• Experience with data management tools.

Responsibilities

• Completes psychosocial assessments for post-hospital care and family support.
• Assists patients in coping with stress related to hospitalization and illness.
• Collaborates with the Clinical Care Management team to determine care level.
• Provides information on community resources and post-hospital options.
• Makes referrals to community agencies as needed.
• Reports suspected abuse/neglect/exploitation.
• Liaison between hospital, patients, and social services.
• Responds to after-hours consultative requests.
