Job summary

Advocate Health in Charlotte, NC, is seeking a part-time Social Worker to perform psychosocial assessments for post-hospital care and support the patient and their family through transitions in care.

You will collaborate with the Clinical Care Management team to determine appropriate care settings, provide community resource referrals, and assist with discharge planning. The role requires strong communication and teamwork in a fast-paced hospital environment.

Qualifications

• Master’s degree in Social Work (MSW) required.
• LCSW preferred; state certification preferred.
• At least 1 year of professional hospital/healthcare experience preferred.
• Experience with data management tools.

Responsibilities

• Performs psychosocial assessments for post-hospital care and supports patients and families.
• Assists patients in coping with stress related to hospitalization and illness.
• Collaborates with Clinical Care Management to determine appropriate level of care.
• Serves as a resource to staff on emotional, social, and psychosocial aspects of illness.
• Provides information and referrals to community resources; makes referrals as needed.
• Reports suspected abuse/neglect/exploitation and liaises with Department of Social Services.
• Responds to after-hours consult requests.
