Job summary

Qualifications

• Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).
• LCSW preferred; state licensure requirements apply if practicing in SC.
• At least 1 year of professional experience in a hospital or health-related setting preferred.
• Expertise with Data Management Tools; adherence to NASW Code of Ethics.

Responsibilities

• Interview patients and social support systems to complete psychosocial evaluations and plans of care.
• Provide psychosocial counseling, support, and education to patients and families.
• Assist with crisis intervention and coordinate with multidisciplinary teams to ensure appropriate care.
• Document psychosocial assessments and maintain accurate case records in medical records.
