Job summary

Advocate Health is hiring a Home Medical Social Worker (PRN) in Union for our Home Medical Social Worker role in North Carolina. The position is part-time and PRN, based on business needs, with a pay range of $30.70–$46.05 per hour.

The role provides counseling, supports patients and families, assesses psychosocial needs, and coordinates care with hospital staff to facilitate safe transitions. A Master’s in Social Work is required, with LCSW preferred.

Qualifications

• Master's degree in Social Work required; LCSW preferred.
• At least 1 year of hospital or health-related experience preferred.
• Adherence to National Association of Social Workers Code of Ethics.

Responsibilities

• Interviews patient and social support system to complete psychosocial evaluation.
• Provides counseling and education to patient and support system.
• Collaborates with multidisciplinary team to plan care and discharge.
• Maintains case records and documents in medical records per standards.
• Reports suspected abuse/neglect and liaises with Department of Social Services.
