Atrium Health is seeking a Part-time Social Work Case Manager for the Charlotte area to perform psychosocial evaluations, counseling, and discharge planning across four locations (Belmont, Concord, Pineville, Charlotte). The role requires scheduling flexibility and travel between sites as needed.

The position emphasizes collaboration with a multidisciplinary team, crisis intervention, and connection to community resources to support post-hospital care and transition.

Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• LCSW preferred.
• At least 1 year of professional hospital or health-setting experience preferred.
• Experience with Data Management Tools.

Responsabilités

• Interview patient and supporting system to initiate psychosocial evaluation.
• Provide counseling and education to patient and support system.
• Collaborate with multidisciplinary team to plan care meeting psychosocial needs.
• Communicate with hospital staff on psychosocial aspects of illness and care planning.
• Identify and intervene in crisis situations using appropriate crisis intervention skills.
• Maintain accurate psychosocial assessments and case records.
