Responsibilities

• Interview patient and family to obtain information about home environment and health history.
• Assess data against medical plan and social services to plan therapy.
• Educate patients to understand and follow medical recommendations.
• Coordinate transportation, durable medical equipment, and alleviate patient anxieties.
• Document cases and maintain records for continuity of care.
