Job summary

Avera McKennan Hospital is seeking a Social Worker to plan, organize and implement social work services for medically and socially complex patients and their families. You will conduct assessments, provide counseling, and coordinate discharge planning within a multidisciplinary team.

Responsibilities include interviewing patients, evaluating care needs, and ensuring access to community resources. EMR documentation and collaboration with Case Management are essential to deliver quality care.

Qualifications

• Master's in Social Work from an accredited program.
• Active Social Worker license in the state of practice.
• QMHP is preferred and may be required upon hire.
• Experience with discharge planning and family counseling.

Responsibilities

• Interviews patients and families to obtain home environment and health history.
• Assesses data against medical plan to plan therapy and safety.
• Advises on social problems and arranges discharge care at home.
• Coordinates care conferences and documents in EMR.
• Collaborates with Case Management for continuity of care.
