Job summary

Responsibilities

• Interview patients and families to assess home environment and health history.
• Evaluate data to plan therapy aligned with medical treatment and resources.
• Educate families on medical recommendations and care plans.
• Coordinate transportation, equipment, and community resources as needed.
• Maintain and update patient records and participate in a multidisciplinary team.
