Job summary

Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• 2 years of healthcare social work experience.
• IL-LSW eligibility and NASW Code of Ethics knowledge.
• Wisconsin social work certification or equivalently recognized license.

Responsibilities

• Conducts comprehensive psychosocial assessments and plans within acute care settings.
• Provides social work services including discharge planning and follow-up.
• Collaborates with multidisciplinary teams for safe, timely care transitions.
• Completes utilization management activities and communicates with payers when needed.
• Advocates for patient involvement and facilitates access to community resources.
