Job summary

CommuniCare Family of Companies is seeking an experienced Hospital Liaison in Indianapolis to drive referrals across our facilities. The role focuses on engaging physicians, discharge planners, and hospital staff to grow census and improve patient transitions.

The ideal candidate has 2+ years of hospital marketing experience, established referral networks, and strong communication skills. This full-time position offers a salary with monthly bonus incentives and a team-oriented workplace.

Job Address: 8275 Allison Pointe Trail Suite 370 Indianapolis, IN 46250 Hospital Liaison The CommuniCare Family of Companies, is currently looking for an experienced Sales/Marketing professional for the position of Hospital Liaison overseeing referrals in the Indianapolis, IN market. This position will support our Indianapolis facilities. We are looking for true marketers who get out, hit the pavement, and bring in referrals! Salary plus monthly bonus potential! The Hospital Liaison is a full time salaried position with salary based on experience. We now offer monthly bonus incentives. JOB RESPONSIBILITIES

• Perform pre-admission assessments of the clinical status of patient referrals and evaluation of patient needs.
• Serve as an educational resource for patients, families, hospitals, and healthcare professionals.
• Actively assist the referring and accepting institutions in addressing requirements of specific patients.
• Monitor marketing activity through CRM software.
