Job summary

Memorial Healthcare System in Pembroke Pines, FL invites an experienced RN Case Manager to join our hospital-based care team. You will coordinate discharge planning, education, and resource utilization to ensure appropriate levels of care and safe transitions for patients and families.

Requires RN from an accredited program and BLS Healthcare Provider certification; minimum 1 year in hospital-based CM or 5 years in healthcare.

Qualifications

• RN license required; hospital-based Case Management experience preferred.
• Basic Life Support (BLS) Healthcare Provider certification required upon hire.
