Formación

• Strongly preferred 5 years of experience in hospital patient care.
• Required 3 years of Hospital Social Work experience.

Responsabilidades

• Collaborate with RN case managers and physicians to address patient needs.
• Identify barriers to discharge and recovery.
• Maintain knowledge of insurance regulations and managed-care concepts.
• Develop discharge plans for high-risk patients.
• Coordinate meetings between care teams and families.
• Provide emotional support during critical situations.
