PAM Health Rehabilitation Hospital of Greeley is seeking a dedicated Case Manager to coordinate inpatient care for a defined patient population. You will review admission records, determine appropriate level of care, and liaise with the multidisciplinary team to optimize outcomes.

Responsibilities include assessing discharge needs, planning transitions, and maintaining payor relationships. Licensure in the practicing state is preferred; BLS certification is required.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Three to five years of inpatient case management experience, ideally in acute/IRF/LTACH.

Responsibilities

• Coordinate interdisciplinary care for designated patient population.
• Review inpatient admission records to ensure appropriate level of care.
• Assess discharge needs and coordinate discharge planning.
• Maintain and enhance payor relationships.
