Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Case Manager to coordinate interdisciplinary care, review inpatient admissions for appropriate resource use, and determine discharge plans for a defined patient population.

You will collaborate with the care team to ensure quality outcomes, manage discharge planning, and maintain relationships with payors. Licensure in the state is preferred, and a current BLS certification is required.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification is required.
• Professional disciplines may include OT, PT, RN, RT, SLP, or social worker.

Responsibilities

• Coordinate care for a designated patient population across inpatient services.
• Review admission records to ensure appropriate level of care and resource use.
• Identify discharge needs and coordinate discharge planning with the care team.
• Maintain and enhance payor relationships.
