PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Case Manager to coordinate care for an assigned patient population. You will review admission records to ensure proper utilization and determine level of care, and collaborate with the interdisciplinary team to plan safe discharges.

In this role, you will identify discharge needs, coordinate the discharge plan, and work to strengthen payor relationships while supporting quality outcomes and efficient resource use.

Qualificações

• Licensure in the state where the hospital resides is preferred.
• Designated case manager roles may include OT/PT/RN/RT/SLP/LSW/BSW/MSW/LCSW.
• Current BLS certification required.
• Three to five years inpatient experience preferred in acute/IRF/LTACH settings.

Responsabilidades

• Coordinate care for a designated patient population across the interdisciplinary team.
• Review inpatient admission records to ensure appropriate utilization and level of care.
• Identify discharge needs and coordinate discharge planning.
• Maintain and enhance payer relationships.
