PAM Health Rehabilitation Hospital of Greater Indiana South is seeking a Case Manager to coordinate interdisciplinary care for an inpatient population. You will review admission records to ensure proper utilization and determine admission level of care, while working with the team to plan discharge.

Requirements include licensure in the state, current BLS, and 3–5 years of inpatient experience in acute settings.

Formación

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.

Responsabilidades

• Coordinate care for designated patient population.
• Review inpatient admission records for appropriate utilization.
• Assess discharge needs and coordinate discharge plan.
• Collaborate with interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
