Job summary

Infinity Healthcare Consulting in Carmel, Indiana seeks a Hospital Liaison to ensure seamless admissions from acute and post-acute care. You will develop relationships with hospital discharge planners and drive referrals to grow census and improve patient transitions.

Responsibilities include maintaining CRM dashboards, responding to referrals on Careport and other platforms, and collaborating with Business Development, Operations, and Transitional Advisors to provide timely, definitive answers.

Qualifications

• GED or High School Diploma required; healthcare experience preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively across departments and manage priorities.

Responsibilities

• Stationed daily at primary account to maximize market engagement and referrals.
• Review and update CRM Dashboard for accurate reporting via Forecast and other platforms.
• Respond to referrals on Careport and other platforms in a timely manner.
