Infinity Healthcare Consulting is seeking a Hospital Liaison to ensure seamless admissions from acute and post-acute care. You will generate referrals, build relationships with hospital discharge planners, and lead initiatives to grow census and market exposure. Expect collaboration with AD Concierge and leadership to drive timely admissions.

The role emphasizes time management, multi-tasking, and detailed patient information handling to deliver the best patient experience across our communities.

Qualifikationen

• Must possess a GED or High School Diploma with 2 years’ experience.
• Preferred associate degree in healthcare, business or similar with 1 year experience.
• Skilled nursing experience preferred.

Aufgaben

• Stationed daily at primary account to maximize market engagement and referral review.
• Review and update CRM Dashboard for accurate reporting and forecasting.
• Respond to all incoming referrals on Careport and other platforms promptly.
• Provide definitive answers for all incoming referrals within one hour or by end of day.
• Collaborate with Business Development and Operations leadership to resolve delays in real time.
• Coordinate with AD Concierge on daily updates regarding pending referrals for Forecast.
• Engage with Transitional Advisors on communication with referrals and admissions on CarePort.
• Ensure timely PCC data input including patient demographics, resident representative, POA and guardian information.
• Prepare weekly reports and participate in Quarterly Business Development meetings.
• Contribute to daily Forecast for team review and assist with census calls and morning standups.
• Oversee Guest Relations and Concierge initiatives, including patient rounding and testimonials.
