Job summary

Infinity Healthcare Consulting is seeking a Hospital Liaison to coordinate admissions from acute and post-acute care, develop relationships with hospital discharge planners, and drive market growth through timely referrals and strategic initiatives.

You will work closely with the AD Concierge and leadership to ensure a smooth patient experience, maintain communication, and support census growth across communities served.

Qualifications

• Must have a GED or High School Diploma with 2 years’ experience; an associate degree in healthcare or business is preferred with 1 year experience.
• Must meet health assessment requirements regulated by the state.
• Must be capable of performing the essential functions of the job.

Responsibilities

• Stationed daily at primary account to maximize market engagement and referral review efforts.
• Review and update CRM Dashboard for accurate reporting via Forecast and other platforms.
• Respond to referrals promptly and provide definitive answers within one hour or by end of day.
• Collaborate with leadership to expedite delays and resolve issues in real time.
• Maintain timely PCC data input with patient demographics and Guardian information.
• Participate in weekly reports and quarterly business development meetings.
