Job summary

Infinity Healthcare Consulting is seeking a Hospital Liaison to ensure smooth admissions transitions from acute and post-acute care. You will generate and manage referrals, build relationships with discharge planners, and collaborate with AD Concierge and leadership to grow census.

This role requires strong time management and multitasking, daily CRM updates, timely responses to referrals, and dedication to delivering exceptional patient experiences in our market.

Qualifications

• Must have a GED or High School Diploma with 2 years’ experience.
• Must meet health assessment requirements regulated by the state.
• Capable of performing the essential functions of the job.
• Must have initiative, patience, compassion, self-motivation, and solution oriented.

Responsibilities

• Stationed daily at primary account to maximize market engagement and referral review efforts.
• Review and update CRM dashboard for accurate reporting via Forecast and other platforms.
• Respond to all incoming referrals on Careport and other referral platforms daily.
• Provide definitive answers for all incoming referrals within an hour or by end of day when appropriate.
• Collaborate with Business Development and Operations leadership to expedite delays and provide solutions in real time.
• Collaborate with AD Concierge on daily updates regarding pending referrals for Forecast.
• Collaborate with Transitional Advisors on communication with referrals and admissions on CarePort and other platforms.
• Input PCC data including patient demographics, resident representative, POA, and Guardian information.
• Prepare Weekly Reports for team review.
• Participate in Quarterly Business Development meetings to grow census and market exposure.
• Complete daily Forecast for team review via Census call and meetings.
• Oversee Guest Relations and Concierge initiatives including patient rounding and testimonials.
