Infinity Healthcare Consulting is seeking a Hospital Liaison to ensure seamless admissions from acute and post-acute care. You will build relationships with hospital discharge planners and drive initiatives that align with market growth, coordinating referrals and admissions through CarePort and related platforms.

You will work closely with the AD Concierge and leadership to drive daily admissions, manage time effectively, and support a positive patient experience across our communities.

Qualifikationen

• Must have minimum GED or high school diploma with 2 years’ experience.
• Must meet health assessment requirements regulated by the state.
• Must be capable of performing essential functions of the job.
• Must have initiative, patience, compassion, self-motivation, and solution oriented.

Aufgaben

• Stationed daily at primary account to maximize market engagement and referral review.
• Review and update CRM Dashboard for accurate reporting via Forecast and other platforms.
• Respond to all incoming referrals on Careport and other platforms timely, daily.
• Provide definitive answers for all incoming referrals within an hour or by end of day if appropriate.
• Collaborate with Business Development and Operations leadership to expedite delays and provide solutions.
• Collaborate with AD Concierge on daily updates regarding pending referrals for Forecast.
• Collaborate with Transitional Advisors on communication with incoming referrals and admissions on CarePort.
• Timely PCC data input including patient demographics and guardianship information.
• Weekly Reports
• Participate in Quarterly Business Development meeting to align initiatives and grow census.
• Complete daily Forecast for Team review via Census call and meetings.
• Oversee Guest Relations and Concierge initiatives including patient rounding and testimonials.
