Infinity Healthcare Consulting in Gainesboro, TN, is seeking a full-time Hospital Liaison to support admissions from acute and post-acute settings. You will generate and manage referrals, develop relationships with discharge planners, and partner with leadership to grow census.

The role requires daily CRM updates, timely responses to referrals via CarePort and other platforms, and collaboration with Business Development, Operations, and Admissions to resolve delays.

Qualificações

• Experience coordinating hospital admissions and referrals.
• Ability to update CRM dashboards and report forecasts.
• Strong communication with discharge planners and leadership.

Responsabilidades

• Stationed daily at primary account to maximize market engagement and referral review.
• Review and update CRM Dashboard for accurate reporting via Forecast and other platforms.
• Respond to all incoming referrals on Careport and other platforms timely.
• Provide definitive answers for all incoming referrals within an hour or by end of day.
• Collaborate with admissions, business development, and operations to expedite delays and solutions.
• Overseeing Guest Relations and Concierge initiatives including patient rounding and testimonials.
• Maintain HIPAA confidentiality and protect resident information.
• Report safety incidents and follow facility policies and procedures.
