Qualifications

• Graduate of an Associate's or diploma program in nursing.
• 1 year nursing experience OR 1 year nursing experience in a specialty.
• License to practice in the State of Connecticut

Responsibilities

• Provides skilled nursing care to hospice clients in their homes or community settings.
• Reviews referrals and initiates contact with client and family.
• Performs nursing assessments based on referral data, client/family interview, and clinical observation.

Education
