Job summary

Enhabit Home Health & Hospice is seeking a qualified caregiver for our branch in Texas. The role focuses on coordinating patient-centered transition plans, aligning with clinical guidelines to improve quality and efficiency of home-based care.

Responsibilities include outreach to patients, families, physicians and facilities, and education about Enhabit's services. Candidates must be licensed, with at least one year in home care, and comfortable using mobile devices.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions.
• Develop patient-centered transition plans applying clinical guidelines to promote quality and efficiency.
• Educate patients and families about provided services as a public awareness representative.
