Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to represent the branch in transitional care activities, educate patients and families, and engage with physicians, hospitals, and community partners to promote Enhabit services.

The role requires graduation from an approved clinical education program, state licensure, and at least one year of home care experience, with strong knowledge of regulatory guidelines and the use of mobile technology.

Qualifications

• Must graduate from an approved school of clinical education.
• Must be licensed in the state where practicing.
• Minimum one year experience in home care.
• Knowledge of federal, state, and local laws governing hospice operations.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and in professional contacts to apprise stakeholders of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols and metrics to develop patient-centered transition plans.
• Serve as a public awareness representative and educate about provided services.
